IN-PERSON- Advance Directives for the State of Vermont- Essential Information for Planning Ahead

This year, give yourself and your loved ones the gift of planning ahead and making your healthcare wishes known in the event of a serious medical situation or at the end of life. In this session, we will review the Advance Directive documents for your state (NH or VT) and discuss how to choose a healthcare decision maker and what will happen if you do not choose someone. We will also look at some additional documents you may want to consider, such as a Do Not Resuscitate order. You will make a plan for completing your documents and making sure they can be easily found by those who may need them.  Participants will have the opportunity to sign up for a free individual appointment to finalize their documents with an Advance Care Planning Facilitator.
